This notice is provided in accordance with Texas law. Consumers have the right to access their health care records, contact the applicable licensing authority, and file a consumer complaint.

Requesting Your Health Care Records

You have the right to request access to your health care records. Requests can be made via email, text, or the online client portal. A Request for Medical Records Form will then be sent to you via the portal to complete. Records will be released within 30 days, unless otherwise noted by the clinician.

File a Consumer Complaint
